SUMMARY OF RESPONSIBILITIES:
Working within the context of a multidisciplinary public health team, the Community Health Nurse (CHN) promotes the health of individuals, families, and community members. The CHN is responsible for: client education, counselling, care coordination, develops, implements and evaluates public health programs and services. The CHN follows Best Practice Guidelines and Public Health Standards to promote health, build individual and community capacity, facilitate access and equity, demonstrates professional responsibility and adheres to Nursing Code of Ethics.
QUALIFICATIONS:
• Completion of a Baccalaureate Degree in Nursing (BScN) from a Canadian University
OR
• Diploma in Nursing from a Recognized Canadian College with a certification in Public Health/Community Health;
• Member in good standing with the College of Nurses of Ontario and Ordre des Infirmieres de Quebec;
• Proof of immunization certificate;
• Certificate in tobacco prevention and cessation practices;
• Must be willing to successfully complete any other training that is required by Supervisor.
