Dear parents and patients:

With some mixed feelings I am announcing the closure of my pediatric practice after nearly 40 years of service to the community, effective August 15th, 2014.

I would like to extend my most sincere feelings of gratitude to each and every one of you – patients and families, who gave me the confidence and honour to take care of your precious children.

To ensure the continuity of your medical care and for your convenience, I highly recommend that you obtain a copy of your medical records for yourself, so that you can easily provide medical information to your family physician or your new pediatrician.

I made arrangement with Records Management Ltd. for the storage and management of my patient medical records. Records Management Ltd. will ensure the security and privacy of your medical records and will provide your family physician and new pediatrician with an electronic or paper copy of your chart upon your request.

Thanks a million to all of you for the beautiful years. Whether you have been with me since I opened my office or you just joined my practice one year ago, I have considered it an honor and a privilege to be of service to you. I have truly enjoyed coming to work every single day and always looked forward to doing my best to take care of your children. I also wish to thank my little friends and their parents from Akwesasne and Cornwall Island for their loyalty and support. You all have been an absolute pleasure and I was blessed to have such an amazing group of patients. I will miss you.
