Description of Position: The RN/Case Manager oversees the delivery of home care services to elderly clients enabling them to continue to living independently at their place of residence within the community of Akwesasne. They do this by assessing the needs of the elderly client, providing information on community based in-home services and programs that may be of assistance to the client, making referrals to appropriate agencies and arranging for the delivery of services. The RN/Case Manager determines program specific client eligibility in accordance with agency guidelines. The RN/Case Manager assists clients and/or their caregiver in completing the assessment for services; collects basic data, obtains the required documents and develops the plan of care to be followed.

Qualifications:  Bachelor’s degree in Nursing, a current unrestricted license to practice as a Registered Nurse in New York State and one year demonstrated experience working in a case management setting.   Must be at least 21 years of age, have and maintain a valid driver’s license with a clean drivers abstract in accordance with the Saint Regis Mohawk Tribe’s Fleet Policy.  Candidate must successfully pass criminal background check, prior to beginning employment.
