Annual Salary: RN 01 $59,445.96 – $78,044.55
SUMMARY OF RESPONSIBILITIES:
Working within the context of a multi-disciplinary public health team, the Community Health Nurse (CHN) will be required to plan, organize, develop and deliver health education and health promotion programs and health intervention programs and/or services to community residents. The CHN will establish and maintain positive working relationships with clients, community leaders, colleagues and representatives of other agencies and service providers. Through home visits and using the nursing process, the CHN will identify clients’ needs, activate the nursing care plan and make referrals through the Case Management Team.
QUALIFICATIONS:
Completion of a Baccalaureate Degree in Nursing from a Canadian University, or a Diploma in Nursing from a recognized Canadian College with a certification in Public Health/Community Health, with 2 -5 years’ work experience in Community Health.
Must have a valid and current registration and be a member in good standing with the College of Nurses in Ontario and Ordre des infirmieres et infirmiers du Quebec (College of Nurses in Quebec) or be eligible to apply for a permit. Must possess a valid Driver’s License and be willing to use personal vehicle during course of employment. Must have and maintain CPR and First Aid Certificates.
