Diabetes Nurse Case Manager

Division: IHS, Diabetes Program Salary: $39,852.80 – 60,652.80    29-1111

Description of Position:  Through a case management approach, the Diabetes Nurse Case Manager provides diabetes self-management education for persons with diabetes and their family. This is met in collaboration with SRMHS medical providers by implementing the Indian Health Service standards of care for program participants with diabetes in the risk reduction and/or prevention of cardiovascular disease.

 Qualifications:  Registered Nurse with a current license to practice in New York State with two years supervisory and clinical experience with skillful knowledge in diabetes, heart disease, case management and community education strategies.   Must be at least 21 years of age, have and maintain a valid driver’s license with a clean drivers abstract in accordance with the Saint Regis Mohawk Tribe’s Fleet Policy.
